Emergency Case Manager
Responsibilities
- Emergency case manager will oversee the transition of patients from emergency department to hospitals’ specialist clinics to primary and community care.
- The Emergency case manager is responsible for the coordination, monitoring and tracking of patients to ensure continuity of care of patients treated in emergency department. In addition, s/he would also be expected to take on ad-hoc projects to help improve the process of healthcare delivery across the care continuum
- Assesses, plans, implements and evaluates care of patients with asthma conditions in Emergency department and EDTU according to updated standards of nursing practice.
- Provides clinical support to the asthma service through counseling of asthma patient and families with regard to diagnosis, treatment and expected outcomes.
- Functions as the resource person on asthma clinical nursing and nursing training matters and issues related to the specialty.
- Assists in first aid treatment of asthma patients and emergency resuscitation
- Collaborates with team doctors and asthma nurses and right siting officers to provide continuum of care
- Provides telephonic service by providing basic advice and escalate as per protocol.
- Function as part of the ED team.
Requirements
- At least a Diploma in Nursing
- Previous nursing experience in hospital will be a plus
- Experience in caring for patients with asthma conditions will be an added advantages
- Willing to be based in Emergency Department
- Willing to work shift and 5.5 days per week
