Emergency case manager will oversee the transition of patients from emergency department to hospitals’ specialist clinics to primary and community care.
The Emergency case manager is responsible for the coordination, monitoring and tracking of patients to ensure continuity of care of patients treated in emergency department. In addition, s/he would also be expected to take on ad-hoc projects to help improve the process of healthcare delivery across the care continuum
Assesses, plans, implements and evaluates care of patients with asthma conditions in Emergency department and EDTU according to updated standards of nursing practice.
Provides clinical support to the asthma service through counseling of asthma patient and families with regard to diagnosis, treatment and expected outcomes.
Functions as the resource person on asthma clinical nursing and nursing training matters and issues related to the specialty.
Assists in first aid treatment of asthma patients and emergency resuscitation
Collaborates with team doctors and asthma nurses and right siting officers to provide continuum of care
Provides telephonic service by providing basic advice and escalate as per protocol.
Function as part of the ED team.
Requirements
At least a Diploma in Nursing
Previous nursing experience in hospital will be a plus
Experience in caring for patients with asthma conditions will be an added advantages
